Care Management Services
Providing care is a hard job. Sometimes having a professional partner to talk to makes it easier when you are looking for resources or making care planning decisions.
Care Manage For All LLC offers three services to assist you in your care work:
1. Consultation
Let us talk by phone about care needs to find ideas or resources you may want to investigate. We will build a care plan together!
2. Education/Referral Services
You have a care plan in place but limited time or energy to research or find services. Let us help you! By phone, our professional staff will listen carefully to you. We will talk about types of resources and investigate referrals or resources specific to the care receiver’s community. We always include helpful tips to support you as you provide care.
3. On-the-Ground Assessments/Visits in the New York State Finger Lakes region and surrounding areas.

Kim Evanoski MPA, LMSW is a private Care Manager/Owner of Care Manage For All LLC serving Upstate New York which includes Ithaca NY, Syracuse NY, Elmira NY, Corning NY, Cortland NY, Endicott NY, Binghamton NY, and Watkins Glen, NY.
Kim feels privileged to work in healthcare, social work, public administration, education and the care management field serving elders, individuals with disabilities, caregivers, and families living in New York State over the last twenty years. She holds a Masters in Public Administration and a Masters in Social Work. Her passion is to promote aging care partnerships, caregiver wellness and education, and build professional collaborations that encourage professional self-care wellness.
